Therapy is a journey of self-introspection, leaning into difficult matters and coming out better as a result. I will be your guide as you walk this path of looking inward to change or let go of those things that don't serve you. It's an uncovering of the good that is within and implementing strategies to maintain a life full of joy, contentment, and peace. I will help you uncover your values (what is important to you) and assist in taking committed action towards those values with skills such as mindfulness, opposite action, emotion regulation, and distress tolerance.

My therapy style is warm and nonjudgmental as I want all of my clients to feel they are in a safe environment when working with me. I believe in treating everyone as an individual and meeting my clients where they are during sessions. I also work with my clients in identifying their individual goals and completing unique and specific treatment plans. During my years of practice, I have identified the benefits of focusing on one's self esteem in order to improve within themselves, specifically focusing on "the here and now".

When people are challenged with changes that affect the ability to manage stressors, there comes a time for positive change and support. I am passionate about focusing on the healing process, whether it involves the challenges of exploring maladaptive behaviors or the triggers of trauma. I help my clients, who include children, adults, and families to find healthy coping mechanisms and self-autonomy which will lead to positive perceptions. This, in turn, creates personal identification and self-worth, thus creating healthy relationships.

Most everything we value flows from the relationship we have with ourselves. Discover all that transforms when you live in a way that reflects compassion toward yourself, and a deeper connection with your emotions, truth and dreams. I am a warm, genuinely caring and intuitive therapist. People I see often say that they feel safe and at ease with me, and deeply understood. Prior to a private practice, I provided therapy at the Departments of Psychiatry at UCSD and Rady Children's Hospital, and at Scripps, and trained other therapists. I use a blend of approaches to therapy and healing, and can offer tools tailored to your needs.

Our mission is to provide quality online mental health and substance abuse services while breaking down the barriers of traditional, office based therapy. Traditional mental health and substance abuse treatment tends to focus on what makes you sick, what you are lacking but at The Mindful Lemon, our clinicians let you sit in the driver’s seat, focusing on your strengths and assets to help you become the best version of you! This can be via traditional talk therapy, medication management, mindfulness techniques, an emphasis on health, wellness and fitness or any combination of the above.

You may have often heard some people say that therapy is for only those with a mental illness or are "crazy". I'm Mark G. Koch and have been a licensed Marriage Family Therapist since 1996, and I can honestly tell you that those type of statements are a myth. Everyone has a view of how the world works and we use the tools we have to deal with our issues, but occasionally there comes a time when a problem arises that we have no answer to. At this point we become "stuck".

Painful experiences can leave us at a crossroads or discouraged about our faith when breakthrough has yet to occur. Have you been feeling “stuck” in negative thoughts? Ruminating thoughts and triggers are common responses to trauma and anxiety. Battling these distressing responses is not only difficult but exhausting. Trying to understand the role faith plays can be confusing. I am passionate about normalizing what you are going through and identifying barriers that are preventing you from living life fully. Everyone responds differently to distressing experiences and therefore have unique needs within the healing process.

I consider myself to be bi-cultural. Having been raised as a first generation American in a Latin family, I can identify with the the mentality of someone living within two "worlds". Transitioning from one culture to another with seemingly different values, beliefs and language. I can easily understand the struggle that maintaining, and existing in both, can sometimes be. I've come to perceive and appreciate it as the gift of having a "bridge" that allows connection to, and understanding of, different aspects of people and life.

We all struggle at times to see our inner strengths and capabilities due to the life experiences which have shaped us. I work in the belief that we each have everything we need to achieve a fulfilled and successful life already present within us; sometimes we just need a little help accessing it. Using a multi-modality approach I encourage my clients to work in the small steps, not the giant leaps, that bring us to self awareness, acceptance, and lasting change.

Obsessive-Compulsive (OCD) Therapists

How long does OCD treatment take?

Obsessive-Compulsive Disorder (OCD) is often treated on a weekly or twice-weekly schedule, depending on symptom severity and the client’s preferences. Some clients may start to see a noticeable improvement in symptoms in as little as six weeks, but it’s also possible, especially in more severe cases, for treatment to take several months to a year before significant progress is made.

What happens if OCD is not treated?

Some people with OCD, especially those with mild symptoms, may find that they are able to adapt reasonably well to life with the condition, even if their symptoms never resolve completely. However, many people, particularly those whose symptoms are severe at the outset, find that their obsessions and compulsions grow more intrusive with time, making it harder to maintain relationships, hold down a job, or navigate the world successfully.

Are there medications for OCD?

Medication can be used to treat OCD. The class of drugs most often prescribed are selective serotonin reuptake inhibitors, or SSRIs, which include fluvoxamine (under the brand name Luvox), fluoxetine (Prozac), and sertraline (Zoloft), among others. In cases of treatment-resistant OCD, other classes of medication, such as tricyclic antidepressants or atypical antipsychotics, may be prescribed. Medication is frequently used in conjunction with therapy.

Can OCD come back after treatment?

Yes. Major life transitions, periods of stress, or other factors can cause symptoms to return or, if they were never fully eradicated, to ramp back up in intensity. For some, these relapses are brief and do not require additional treatment; identifying specific triggers and practicing the skills learned in therapy can help speed their course. For others, returning to therapy for a brief period can help address the underlying stress and strengthen coping mechanisms.
