Kelly Theodore is a dual certified psychiatric and family nurse practitioner who has been practicing in Wisconsin since 2015. She works with adults ages 18-65+ for psychiatric assessments and medication management. Kelly is passionate about working with people to reach their full potential by helping them reconnect to their mental and physical health. Her care philosophy integrates mental and physical health for a highly individualized treatment in an all-inclusive, non-judgmental and empathetic space.

I am a Psychiatric Mental Health Nurse Practitioner and have been providing services for in home and treatment settings. I see adults, and seniors and address addiction/substance abuse, anxiety, depression, and traumatic disorders. I like to intervene as part of the treatment team as soon as possible to veer outcomes away from substance use disorders or legal troubles as adults. Dr. Vanden Noven is a Board-Certified psychiatrist in both General Psychiatry and Child and Adolescent Psychiatry. She graduated from medical school at the University of Toledo in 2007 and then relocated to Milwaukee for psychiatric training. She completed her general psychiatry residency at the Medical College of Wisconsin in 2011. She then completed an additional two years of fellowship training in child and adolescent psychiatry, also at the Medical College of Wisconsin. Dr. Vanden Noven treats children, adolescents, and adults with a wide variety of psychiatric conditions.

Heather D. Talley is a Licensed Professional Counselor (LPC) as well as a Clinical Substance Abuse Counselor. She received her Master’s Degree from Springfield College in Community Counseling. She has been providing therapeutic services since 2013.

In her spare time she enjoys spending time with family, pets and travelling.
